Change of Building Request

Below are forms to request a change of building assignment for students in kindergarten through grade 8.  All students must be registered in Carlisle Area School District prior to parents/guardians completing and submitting a form.  This process is to assist families with before and after school childcare needs and to keep siblings in the same school when one is specially placed by the district.

A new form needs to be completed each school year.

The form is a fill-able PDF and should be completed and returned electronically as an email attachment.

Complete one form per student (siblings require separate forms).

If the request is granted, it will be the responsibility of the family to provide on-time transportation for the start and end of each school day.

Babysitting/Childcare transportation request forms are available in the PowerSchool parent portal and need to be completed annually.

Forms for the following school year are reviewed over the summer and decision letters are sent in August.
