PIAA and Recertification through PowerSchool Parent Portal

Is your child interested in playing a Winter sport?

A student-athlete is required to have the following:

  1. PIAA Form - Completed 4-part PIAA form prior to participation in your child’s first athletic season of the school year.  This form must only be completed once per year through the PowerSchool Parent Portal.  Parents and students must complete the forms together.
  2. Physical Exam- Completed by physician on/after May 1, 2024 on PIAA form (Section 6) Click here for the form
  3. IMPACT Test - Current within 2 years of the 1st practice date (November 14, 2024).  Click here for more information.

 
ALL forms are due to the Athletic Trainer's Room by 4:00 PM on
Wednesday, November 13, 2024.  
 
Failure to have all three requirements completed by the above due dates may result in not be allowed to participate on the 1st day of practice.

New for Winter-season vs. Participated in a Fall-season sport?

IF a winter-season sport is the first sport your child is participating in for this school year:

  • Logon to the PowerSchool Parent Portal or access the PowerSchool App.
  • Select the student-athlete.
  • From the Forms menu (More -> Forms), select ‘2024-25 PIAA Athletics’ form.
  • Complete the four-part form in its entirety and hit SUBMIT.
  • If you are obtaining a physical from an outside facility, you will be able to print the health history and physical exam paperwork to take to the doctor. 

IF your student participated in a fall-season sport and wants to participate in a winter-season sport:

  • Logon to the PowerSchool Parent Portal or access the PowerSchool App.
  • Select the student-athlete.  
  • From the Forms menu (More button -> Forms), select ‘2024-25 PIAA Athletics Winter Re-Certification’ form. 
  • Complete the form and hit SUBMIT.

** Parents of students who are NOT NEW to this District this year, MUST have COMPLETED the District’s 'Returning Student Forms’ (PowerSchool Parent Portal) and ALL parents must have the 'Science: Parent Safety Form" completed before accessing athletic forms.  Students will NOT be able to attend a try-out for a sport until all forms are completed.  Contact Stephanie Douglas at [email protected] if you need assistance with your PowerSchool account or completing forms. **

CASD will continue to provide sports physicals.
